We will need the following documents from you as soon as possible (the links to these documents are on the website under “Forms”) (if the symbol next to the document is next to your name, we have received it):
- Signature Page of the Player/Participant Policies #
- Medical Release Form (yes, we need another copy)^
- BIP Waiver – initialed and signed by BOTH player and parent *
- Copy of the inside cover of Player’s Passport (with picture and document information) %
