Congratulations on an outstanding SCVA HP tryout. On behalf of the SCVA, I would like to inform you that you are invited to be a member of the SCVA High Performance Team traveling to Des Moines, Iowa.
Twelve athletes have been chosen to train with the SCVA HP Youth Team. Training will be July 13-14 (6-9pm); July 15 (12-3pm) and July 18-19 (2-5pm) at the ASC.
Please log on to your SCVAHP ACCOUNT and complete the additional required information (uniform sizes). Create an account if you were a walk up registrant and add your daughter.
You will also need to print, complete and mail the signature page of the Participant Policies, another copy of the medical release form, and a copy of your daughter’s birth certificate to the SCVA at the address below. The Participant Policies and Medical Release can be found here SCVA HP FORMS Please submit these documents to me no later than May 15.
Deadlines:
April 22, 2015: Confirm or Decline your position to
valerie@scvahighperformance.com. When responding please include your name in the subject box (i.e., Valerie Cowan Youth).
Once you have accepted your position, I will add you to the appropriate team, on your SCVA HP Account, which will then show the schedule of fees due. You will then have the ability to use a credit card option for those payments through the website, however there will be an additional $50 service fee added to the fees (even if only a portion is paid by cc). If you wish to pay in full via credit card, please notify me as I can make that a payment option.
April 30, 2015: Non Refundable Deposits are due - $750 checks payable to
"SCVA"
May 30 and June 30, 2015: Payment of $550 checks payable to
"SCVA"
Refer to your SCVA HP Account regarding fees. I will update what has been received in the system beginning April 30.
