Congratulations on an outstanding SCVA HP tryout. On behalf of the SCVA, I would like to inform you that you are invited to be a member of the SCVA High Performance Teams competing in Ft. Lauderdale.

____ YES _____ NO: Training will be July 11-12 (2-5) ; 13 (12-3); 16-17 (2-5) at the ASC (tentative). Cost of participating in this event is $1850 which includes all expenses.. We will fly out on July 18 returning the morning of July 24. All information will be on the website.
I will need your response and the following Mizuno size information no later than Monday, April 25, 2016.
We are using MIZUNO this year, so please include the following information with your response:
Jersey size: XS S M L XL
Spandex: XS S M L XL
Shoe size (Women’s Wave Lightning 2): ____
Warm up jacket: XS S M L XL
Warm up pant: XS S S-Tall M M-Tall L L-Tall XL XL-Tall

You will also need to print, complete and mail the signature page of the Participant Policies, another copy of the medical release form, and a copy of your daughter’s birth certificate to the SCVA at the address below. The Participant Policies and Medical Release can be found here SCVA HP FORMS
Deadlines:
April 25, 2016: Confirm or decline to valerie@scvahighperformance.com . When emailing your response, please include your name in the subject line of the send. Example Email Subject line: Betty Blocker Select/Youth. Please include all requested size information with your response!
Once you have accepted your position, I will add you to the appropriate team, on your SCVA HP Account, which will then show the schedule of fees due. You will then have the ability to use a credit card option for those payments through the website. There will be a $50 service charge when using the credit card option.
May 1, 2016: Deposits are due - $750 checks payable to "SCVA"
June 1 and July 1, 2016: Payment of $550 checks payable to "SCVA"
Once you are assigned to a team, there will be the option to pay the deposit with two follow up payments OR to pay in full. Once I recieve your payment, I will make the necessary adjustments to the amount owed. (currently having program issues to assign these to the players).
